Step 1: Go to Form builder >> Dashboard

Step 2: Click on Add new form and Click on Sticky form

Step 3; Give a name to sticky form and Choose the field by using Drag and drop form editor

Step 4: Click on Publish, you will get the Embed code and paste it as per your need
